This form should only be used when a district would like to enroll an out of district student outside of the Schools of Choice windows as articulated in the Montcalm Area ISD Cooperative SOC Program document.
SOC Provision of Service Agreement
The SOC Provision of Service Agreement is to be used when a SOC application has been received for a student who resides in a county other than Montcalm County and has an Individualized Education Program (IEP). This agreement is for school administration only and must be obtained prior to acceptance under Schools of Choice. This requirement complies with IDEA (PL 108-446).
